*Position Title:* *Substance Use Disorder (SUD) Family Group Facilitator*
*Supervisor:* Director of Clinical Services

Transitions, Inc. is looking for professionals to join our team with opportunities within Residential, Intensive Outpatient and Partial Hospitalization Services. Transitions offers an environment for individuals to understand and treat the complex nature of Substance Use Disorders as well as co-occurring mental health disorders.
*Qualifications:* Bachelor’s degree in counseling, social work, psychology, or a related field required. Master’s degree preferred. Prior experience facilitating family groups or working with individuals and families affected by substance use disorders. An active Kentucky License including CSW, LCSW, LPCA, LPCC or LPA, Certified by the state of Kentucky as a Certified Alcohol and Drug Counselor (CADC) or other relevant credentials preferred.
*Position Summary:*
Responsible for leading and facilitating family group sessions for individuals impacted by substance use disorders. This role includes scheduling and coordinating family sessions, ensuring appropriate documentation, and fostering a supportive and constructive environment. The facilitator will utilize strong communication skills, active listening, and conflict resolution techniques to guide discussions and encourage healthy family interactions.
*Job Responsibilities:*
Ø *Facilitation & Group Leadership:*
· Lead structured family group sessions, guiding discussions on topics related to addiction, recovery, family dynamics, and communication.
· Foster a supportive, non-judgmental space that promotes open dialogue and constructive interactions.
· Use evidence-based approaches to facilitate discussions and educate families on addiction, coping strategies, and healthy boundary setting.
· Recognize and address conflict within group discussions, using de-escalation and mediation techniques as needed.
Ø *Scheduling & Coordination:*
· Schedule family sessions and coordinate with clients, family members, and clinical staff to ensure availability and participation.
· Communicate with therapists, case managers, and other treatment team members to ensure continuity of care.
· Adjust schedules as needed to accommodate family needs and program requirements.
Ø *Documentation & Compliance:*
· Maintain accurate and timely documentation of family sessions in the Electronic Health Record (EHR) system.
· Ensure all documentation meets clinical, ethical, and confidentiality standards.
· Provide reports on group progress and escalate concerns to the clinical team as needed.
Ø *Communication & Conflict Resolution:*
· Utilize active listening and effective communication strategies to engage families in meaningful dialogue.
· Address and de-escalate conflicts between family members, promoting problem-solving and constructive interactions.
· Encourage collaboration among family members to strengthen support networks and improve recovery outcomes.
Ø *Training & Professional Development:*
· Participate in staff meetings, supervision, and training sessions to stay informed on best practices in addiction treatment and family therapy.
· Stay up to date on current research and strategies for working with families affected by substance use disorders.
· Performs other related duties as assigned.
Job Type: Full-time
Pay: $55,000.00 - $65,000.00 per year
