Patient Specific Medications Coordinator

Cleveland Clinic’s inpatient pharmacy areas support distributive and clinical pharmacy services provided by staff pharmacists, clinical pharmacy specialists and pharmacy technicians, supporting 60 nursing units, 80 operating rooms, 4 PACUs, and 10 cath labs, accounting for over 1,440 inpatient beds. As a Patient Specific Medications Coordinator, you will promote an integrated and coordinated approach to managing the patient specific medication process, serve as a liaison to patients and clinical teams on the patient specific medication process, and proactively reach out to patien ts and external specialty pharmacies to coordinate refills.  

A caregiver in this position works days/evenings/nights/weekends between 12:00am – 11:30pm.  

A caregiver who excels in this role will:  

  • In partnership with pharmacy leadership team, develop standard operating procedures that effectively support the care and financial impact for patients requiring patient specific medications.  

  • Utilize all pharmacy systems to review patient profiles , enter changes, and monitor treatment schedules .  

  • Monitor and track the status of patient specific medications and communicate with the pharmacy, physician and clinical team.  

  • Track and monitor data analytics and databases related to all new patients being treated at main campus pharmacy locations with patient specific medications.  

  • Capture data queries that will drive decisions and process improvement for treatment delivery as well as collaboration with payers.  

  • Document all patient/client/provider contact in appropriate computer systems.  

  • Guide patients through appropriate patient specific medication obtainment process.  

  • Identif y , evaluate, and resolve patient complaints related to patient specific medication process.  

  • Communicate with patients in a professional and customer-friendly manner in order to schedule the delivery of refilled patient specific medications.  

  • Coordinate with clinical team in obtaining information and documentation for eligible patients.  

  • Clarif y medications and dosages for patient specific medications to facilitate patient specific medication renewals.  

  • Forward adjustment requests to Pharmacy Billing team for patient specific medications when applicable to correct erroneous charging.  

  • Educate caregivers and a dministration to assist in understanding of processes and procedures surrounding patient specific medicat ions.  

  • Coordinate with regional treatment sites to ensure patient specific infusion medications are available at the site of care.  

  • Implement, manage, and maintain inventory systems to order, control, expedite and inventory patient specific medications to enhance the fiscal management of the department.  

Minimum qualifications for the ideal future caregiver include:  

  • Associate’s degree.  

  • Certified Pharmacy Technician ( CPhT ) verified through the Pharmacy Technician Certification Board (PTCB).  

  • Registration as a Certified Pharmacy Technician, Registered Pharmacy Technician or Pharmacy Technician Trainee with the Ohio Board of Pharmacy. Registration as a Certified Pharmacy Technician with the Ohio Board of Pharmacy is required within 6 months .   

  • Three years of pharmacy technician experience in a healthcare environment.  

  • A high school diploma/GED and five years of progressively responsible healthcare or pharmacy-related experience may substitute the education requirement.  

  • Computer experience and ability to navigate multiple systems simultaneously.  

  • Knowledge of insurance coverage, limitations, requirements, eligibility, pharmacy formulary, prior authorization, coordination of benefits.  

Preferred qualifications for the ideal future caregiver include:  

  • Bachelor’s degree.  

  • Completion of a formal pharmacy technician training program.  

  • Experience working and communicating with multiple healthcare disciplines to coordinate care for patients.  

  • Experience with solving issues/problems related to coordinating prescription processing and delivery with external pharmacies.

  • Normal or corrected vision.
  • Extensive sitting for prolonged periods of time.
  • Ability to clearly communicate verbally by phone and in person.
