Pathways Community Health Worker – Posting #27396

Hourly Rate: $21.75

Position Summary:

Full-Time position available for a Pathways Community Health Worker for our Care Management team in Vancouver, WA. This position works closely with all clients, community organizations, and other departments. This culturally and geographically connected individual serves as a link between underserved communities and existing community resources. Candidates with a background in community outreach or case management are preferred. Knowledge of community resources is preferred. Through home visiting, this individual assists clients in overcoming barriers to health, social services, education and employment. Candidates who are bilingual in Russian or Spanish are highly preferred.

Driving to meet clients in homes, clinics, and hospitals is required. This person must have a valid driver license, proof of auto insurance and a vehicle safe for daily use. Prior to hire this person must submit a driver’s abstract demonstrating that they are safe to drive as deemed by Sea Mar’s Health and Safety/Compliance Officer. Within the course of employment, a current driver’s abstract may be requested. 

Duties and Responsibilities:
  • Be a patient advocate.
  • Conduct standardized patient in-take and risk assessments.
  • Establish a trusting relationship with the target population, referred clients, and community partners.
  • Identify and assist clients with client identified health and social service goals as appropriate.
  • Assist in the development of strategies to address or eliminate barriers to care.
  • Work collaboratively with the Program Manager, PCP, and other assigned personnel to make sure that all relevant client needs are identified and addressed.
  • Participate in on going assessment of the client and family needs to determine if expected outcomes are reached and to ensure implementation and/or adjustment of the service plan.
  • Providing ongoing follow-up on treatment on PCP plans utilizing basic motivational interviewing and goal setting with clients/families.

Requirements and/or Experience:

  • High School Diploma or Equivalent required.
  • Community outreach or case management experience preferred.
  • Experience working with people from various backgrounds required.
  • Experience working with individuals with substance use disorders, homelessness, chronic mental illness, or people in crisis preferred. 
  • Maintain a clear understanding of the Federal, State, and community resources available.
  • Demonstrate the ability to recognize potentially dangerous situations to self in the clinic,
    office, home and community settings and to take appropriate action.
  • Prefer one year paid or volunteer experience providing medical, social service or behavioral health case management or care coordination.
  • Must have current CPR certification and maintain throughout employment.
  • Bilingual English/Spanish or English/Russian is preferred, but not required.
