Job Description

Overview The Corps Case Manager (Shelter) will be primarily responsible for the effective and efficient administration of social services programs, including a focus on Cornerstone Shelter, developing case plans for clients, and providing assistance to carry through case plans at the Montclair Citadel Corps.   • Perform intakes, interviews, and assessments of clients’ Homeless Prevention needs and perform follow up case management with individuals and families to determine appropriate supportive programs available.• Maintain accurate, confidential, and updated case records in accordance with The Salvation Army record-keeping policies and procedures.• Provide comprehensive case management for all shelter clients including, but not limited to, assessment of client needs, developing a plan of action utilizing a trauma-informed approach, and identifying internal and external programs that would support their pathway to self-sufficiency.• Complete home visits and inspections as needed for various program requirements.• Coordinate services with partner agencies to ensure a holistic service approach for all clients.• Ensure all homeless program participants are entered in the Coordinated Entry System• Work with the Drop-in Center Coordinator (as needed) to develop social and educational programs and work with clients to help connect with appropriate support services that will result in establishing housing stabilization plans.• Conduct intake and establish case plans for shelter clients and provide case management that includes a case plan to return to stable housing.• Establish and strengthen solid community collaborations with outside agencies, and serve as a client advocate, enhancing a collaborative relationship to maximize the client’s ability to make informed decisions.Program Management• Manage Cornerstone Shelter staff and coordinate scheduling.• Provide emergency coverage at Cornerstone Shelter when needed.• Ensure the accuracy and timely submission of records and reports in applicable databases for all community programs and funding sources as requested by the Corps Officers.• Assist with coordination and oversight of volunteers for social service programs, both ongoing and special events
  • Assist with special events planning, seasonal assistance, and implementation, i.e., Angel tree Program.• Assist Corps Officer(s) with seasonal social service ministries during the holidays, particularly with the Angel Tree client applications.
Grants Management and Administration • Provide task supervision to BSW students fulfilling internship requirements at TSA Montclair Citadel and provide all necessary communication and documentation for the college/university.• Refer clients in need of pastoral care to Corps Officers for follow-up and partner with Officers to link families and individuals to other services and programs provided by the Corps.• Coordinate homeless prevention and shelter services with partner agencies.• Oversee Intensive Case Management and Prevention services through various funding sources and assist with other programs as needed.• Participate in Street Outreach events and activities in Montclair and Greater Essex County, including the annual Point in Time survey and Homeless Connect events.• Maintain client case records in the HMIS database.• Produce regularly scheduled reports for all funding sources as required and ensure that criteria for each is met.• Attend Salvation Army and other program-mandated training such as HMIS as required for licensing or funding compliance.• Participate in a CEAS/CoC subcommittee.• Assist in the upkeep of the website and social media as needed.• Represent The Salvation Army as required at Social Service events.• Relate professionally to colleagues, the public, and all current and prospective clients.• Arrange workshops, lectures, and training to clients, families, employees, and community groups as needed.• Uphold the standards and principles of The Salvation Army’s mission and ministry.• Other duties within the scope of the role as assigned by supervisors. Qualifications
  • Bachelor’s degree in social work, Psychology, Counseling or Human Services related field from an accredited institution; MSW preferred.• Possess a minimum of two years’ experience in counseling and case management, preferably in the area of homeless services.• Ability to maintain confidentiality of interactions with clients and fellow employees.• Proficiency in Microsoft Office programs, including but not limited to Word, Excel, & Outlook.• NJ Driver’s License with a clean driving record• Must demonstrate sensitivity and compassion to the needs of the people we serve.• Must understand and support the mission of The Salvation Army.
  • Bilingual in English and Spanish preferred.• Strong organizational skills, attention to detail, and strong written and verbal communication skills• Self-motivated and able to work well under limited supervision.• Have knowledge of Essex County and local community resources.• Strong interpersonal skills and ability to work as part of a team.• Excellent written and verbal communication skills and ability to establish rapport.
