Community Health Worker - Health Promotion Services

Worksite Location: Hood River

Work Schedule: Full-Time; 40 hours per week; Monday-Friday; Some evenings and weekends

Wage Range: $22.91 - $25.16 per hour

Bilingual Differential Pay: $0.50 per hour worked

Reports to: Health Promotion Services Program Supervisor

Who are we looking for?

The Community Health Worker (CHW) assists program participants in accessing services to improve or promote health and wellness. Assisting participants may include navigating program pathways, documenting progress, and empowering program participants through supportive relationships.

What you will do…

  • Research and track policies that affect the immigrant and refugee community, by following local, state, and federal legislation
  • Compile resources and facilitate a coordinated organizational response
  • Serve as a resource for TNDI and broader community for immigration related information
  • Serve as a resource to community members, immigrants and refugees seeking guidance on immigration cases through connecting them to resources and helping them navigate a changing political landscape, making sure they get the resources they need.
  • Facilitate community and internal meetings as needed to keep community members and staff informed and up to date on policy that affects vulnerable communities
  • Build strong relationships with community members / families / households by offering care that fits their culture.
  • Work with community members / families / households to help them manage their own care and make decisions together.
  • Find out what services are needed, what’s missing, and what’s being duplicated, and work with other organizations to fix any issues.
  • Speak up for community members / families / households to help them use services they need.
  • Make sure any service gaps are noted and communicated clearly.
  • Use and improve local resources that are already available.
  • Build good relationships with other community health workers and partners.
  • Create connections with health care and social service providers.
  • Attend all required training and meetings for community health workers.
  • Give feedback to help improve programs.
  • Complete enrollments, assessments, and follow up with clients
  • Assist in planning and running educational workshops and activities.
  • Other duties as assigned by supervisor.

Education, Experience, and Requirements…

Required:

  • High School Diploma or equivalent
  • Bilingual in Spanish and English
  • General understanding of U.S. immigration policy
  • Community Health Worker certification or ability to obtain one within 90 days of employment
  • Valid driver’s license with auto liability insurance
  • Successful completion of a comprehensive background check
  • Experience working with Microsoft 365 and Zoom, or willingness to learn
  • Experience working with vulnerable populations

Preferred:

  • Experience working in resource navigation for immigrants and/or refugees or related field
  • Understanding of social services and resources in the Mid-Columbia Gorge region
  • Experience using social media platforms, or willingness to learn
