Community Health Worker (Court Compliance) - HHS


Job Description

PURPOSE SUMMARY. Under the supervision of the designated Program Administrator in the Court Compliance Division, this position provides community support services, client services and care navigation services to people involved in or at risk of becoming involved in the criminal justice system. 

 

ESSENTIAL DUTIES:

1. Resource Navigation: 



  • Work with clients to identify health, social, economic, and educational needs, by conducting client assessments and ongoing interactions with the clients.



  • Educate clients about treatment programs, public assistance programs, and other resources to improve access to care and support recovery.



  • Link clients to resources in the community to meet needs through referrals and warm hand-offs with community organizations. May include coordinating transportation to appointments or attending appointments with clients.



  • Understand and promote person-first, recovery-orientated approaches, and services.



  • Document clients’ progress and maintain proper records to track adherence to court orders or engagement with care plans, as well as maintain confidentiality. 



  • Assist clients with access to healthcare and social services in an appropriate setting, and advocate for clients if needed.


 

2. Client and Community Engagement:



  • Schedule home or community visits to engage with clients and assess appropriate living conditions and support client engagement.



  • Conduct regular face-to-face meetings with clients, attend community events, participate in professional training, and build relationships with community providers.



  • Network with community-based organizations to identify available resources and develop partnerships to benefit clients.



  • Identify and address needs and barriers to support access to services and recovery.



  • Maintain client-centered focus and non-stigmatizing communication, focusing on harm reduction when needed.



  • Create plans to reengage clients that are most essential to serve, but may be unable to access services, including motivational interviewing and brief interventions.


 

3. Communication:



  • Meet with the program staff and supervisor on a regular basis.



  • Work within a multidisciplinary team member of the Court Compliance Staff and other HHS staff.



  • Represent the department programs at meetings or conferences as requested by the division manager or program administrator.



  • Assist with the development of media messages and campaigns as appropriate.



  • Facilitate the prompt and accurate submission of information reports or data as required by funding agencies, division manager or program administrator.



  • Compose correspondence, reports, spreadsheets, requisitions and other documents.



  • Complete necessary administrative forms or uses established software for accurate and complete data collection.


 

ADDITIONAL DUTIES.

Other duties may be assigned. Position requires, on occasion, work beyond normal business hours and/or extended hours, nights and weekends.

 

QUALIFICATIONS.

A. Education. High school diploma or GED.

 

B. Experience. Four years full time experience in health care, health promotion, outreach, community, and customer service activities or case management. Bilingual (English/Spanish) is required. 

 

C. Education/Experience substitution. In accordance with County policy.

 

D. Licenses/Certifications. Valid driver’s license. Must maintain a satisfactory driving record in accordance with County policy. Certified as a Peer Support Specialist, Community Health Worker or Family Peer Support Specialist is preferred.

 

E. Other (e.g., post-offer medical exam, polygraph, background check, driver’s license record, etc.).  Must pass background and driver’s license check.

 

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED BY THE POSITION :

Must have knowledge of : Peer support and recovery-oriented support; healthcare and social services systems; motivational interviewing techniques; community-based education principles; and general knowledge of community resources.

 

Must have skills to : Operate general office and computer equipment; be proficient in computer software to include Microsoft Office Suite; maintain accurate records; and communicate effectively in verbal and written form with others.

 

Must have ability to : Work independently; observe clients, assess client’s demeanor and living conditions, recognize behavior changes, and take effective action; prioritize and multi-task; problem solve; interact with a wide range of people; develop collaborative partnerships; utilize referral networks within the local health care system; and maintain confidentiality; communicate effectively both orally and in writing.

Compensation Range

$16.73 - $26.76
